Penis augmentation surgery, or phalloplasty, involves cosmetic surgery to reconstruct the penis. Penis enlargement can add length and girth, helping men to feel more self-confident about the size of their penis.

The first real penis augmentation surgery was performed by the Russian surgeon, Nikolaj A Bogoraz, in 1936 and involved the use of rib cartilage as a penis implant. More recent techniques became accessible to the public in the early 1990s. Since then, the method has continued to advance, reducing risks and maximising the benefits of the procedure.

Following penis enlargement plastic surgery, the penis should appear longer and/or wider, allowing most men to gain 25mm in length or circumference when the penis is flaccid. Please note that the penis will not be longer when erect. Also, if fat transfer techniques have been used to ‘fatten up’ the penis, this is not permanent and will disappear over time.

Are you an appropriate candidate for a phalloplasty?

Despite being reduced in size, a smaller than average penis will still perform as normal. Even during sexual intercourse, there is more to sexual satisfaction than the depth of penetration. Nevertheless, modern society has become preoccupied with the size of the penis, and a smaller than average penis can lead to a lack of self confidence, especially in situations such as communal changing rooms or with a sexual partner.

As one man in his 20s put it, “I knew all the stuff about ‘it’s not the size that matters it’s what you do with it’ but as soon as it entered my head that a woman might think that my penis was small I lost all self confidence, which in turn led to a poor sexual performance. So, despite the theory, the practice was that it did affect my sexual performance, which is why I chose to have a penis enlargement.”

Before undergoing penis enlargement surgery, some surgeons insist that their patients have counselling to ensure they are approaching the operation in a rational manner. During such counselling, it may be suggested that you consider non-surgical alternatives such as shaving off your pubic hair or slimming (if you are overweight), both of which will make your penis appear larger.

Undergoing penis augmentation surgery

Penis enlargement cosmetic surgery involves either or both of two separate surgical procedures. In some procedures, it may be necessary to include circumcision as part of the surgery. To enhance the length of the penis, the surgeon will make an incision at the base of the penis and cut the ligament that attaches it to the body. This will result in the penis extending further out from the body. So, nothing is actually added to the penis, it is simply ‘loosened from its moorings’, allowing more of the penis to stick out from the body.

The girth of the penis can be increased by adding fat beneath the skin. This can be achieved by either sewing on strips of fat (dermal fat grafting) or by injecting fatty tissue into the penis (fat transfer).

After the operation, your penis will be wrapped in surgical dressings that will need to be replaced at regular intervals. You should take a week off work to recuperate, especially if both procedures have been carried out simultaneously. Your surgeon may suggest some postoperative physical therapy to maximise the gains from your penis enlargement surgery.

Maintaining hygiene around the treated area is essential, particularly during the healing period. Avoid showers, using a sponge or washcloth instead. Sutures will be removed at around 10-14 days, unless dissolvable sutures have been used. Avoid swimming and other physical activity for 1 month after the operation, and wait at least 6 weeks before recommencing sexual activity. As one phalloplasty patient said, “After my phalloplasty I was quite swollen for a few days and there was some mild pain. However, basic pain medication was sufficient to alleviate any discomfort, and I followed all the advice from my surgeon to encourage healing, then after about 1 month I felt completely normal again.”

As with all cosmetic surgery, there is some degree of risk associated with the procedure, which the plastic surgeon will explain during your initial consultation. These include, less sensitivity, more sensitivity, asymmetry and less erect erections. However, when performed by a qualified plastic surgeon, and when the patient complies with all pre and post operation advice, the results are typically successful.

Who should carry out a Phalloplasty?

Only fully trained and qualified surgeons should perform penis enlargement surgery. The surgeon should possess the relevant expertise after experience operating in an establishment registered with the Healthcare Commission (in the UK). Private costs for the procedure range from £3,500 to £5,000 for lengthening and £1,000 to £2,500 for increasing the girth. The price for the combined operation is £5,000 or so.
